Skilled Nursing at Home

RN- and LVN-led nursing care at home, including wound care, medication management, IV therapy, and post-hospital recovery.

What is Skilled Nursing?

Skilled nursing is medically necessary care provided by a registered nurse (RN) or licensed vocational nurse (LVN) in the patient's home. It is typically ordered by a physician and is most often covered by Medicare Part A when the patient is homebound and requires intermittent skilled care. Our nurses provide wound care, IV therapy, injections, medication reconciliation, catheter care, ostomy care, and ongoing assessment of medical conditions like diabetes, congestive heart failure, and COPD.

Frequently Asked Questions

Does Medicare cover skilled nursing at home?

Yes. When a physician certifies that the patient is homebound and needs intermittent skilled care, Medicare typically covers skilled nursing visits, physical therapy, occupational therapy, and home health aide services.

How often will a nurse visit?

Visit frequency depends on the care plan. Some patients need daily visits for wound care or IV therapy; others need one or two visits per week. The plan is set by the physician and reviewed regularly.
